Within our Walls: Community Health Needs Assessment

Every three years, CHI St. Alexius Health Carrington conducts a Community Health Needs Assessment (CHNA) to better understand health and well-being at an individual and community level. We need your input to help us understand the needs of our community and identify how CHI can best meet those needs.

As part of our needs assessment, we are conducting a survey to help identify health needs in our community. Those identified needs will inform the development of a strategic plan to help CHI meet our community's health needs.

Participation is voluntary, results are completely anonymous, and responses are confidential. Findings from the survey will be included in our community health needs assessment to be released and shared with the community next year.
